Personal Information Inquiry

Under the Japanese Personal Information Protection Law, Olympus Group Companies in Japan (except for listed companies with their own privacy policies in place) accepts your request (or the request filed by your agent) for access to, or similar matters with respect to, your Personal Information held by them.1

Please note that this inquiry can only be made to inquire about your personal information that Olympus Group Companies in Japan (except for listed companies with their own privacy policies in place) possesses.

We only accept requests made in writing so that we can effectively verify your identification2. You may initiate the request process by printing out and completing the appropriate forms provided below. You may submit the completed form, together with your identification (plus the appropriate fee if the request is for access to your Personal Information) to the Personal Information Inquiry Desk by postal mail. Olympus will, upon receipt, forward the request to an appropriate Group company for necessary procedures, and will reply to you by certified mail3.

note 1: Request for access to, or similar matters with respect to, your Personal Information held by them" refers to: requests for access; for amendment; addition; deletion; discontinuation of use; elimination; discontinuation of provision to third parties; of Personal Information held by Olympus Group Companies in Japan (except for listed companies with their own privacy policies in place)

note 2: Customers who have completed online user registration or who are members of Olympus Zuiko Club may make such requests online by going to the website page.

note 3: We will mail our reply within three weeks of the receipt of your request. We will notify you if we need more than three weeks for investigation.

Please read the following before making an inquiry:

Documents you need to submit

1. Request form
2. Identification for verification that the Personal Information in question does indeed pertain to the individual who is making said request
(Either (1) or (2) must be provided)
(1) Photocopy of a valid drivers' license or a valid passport: One copy
(2) Photocopy of pension card or of health insurance card together with certificate of residence: One set
3. Fee (if a request for access is made): 1,600 yen (in fix-amount postal money order)

Request by an agent

If the request is filed by an agent, the following must be furnished, in addition to the documents specified in (1) through (3) above:

1. For agents with a Power of Attorney: Documents providing evidence that the party making a request has a Power of Attorney from the person to whom the Personal Information relates (Both (1) and (2) below must be furnished)
(1) Power of Attorney (with the seal of the grantor) One
(2) Certificate of a Grantor Seal-Impression One

2. For statutory agent: Document providing evidence that the said agent is legally entitled to represent the individual in question. One

Fee

A Fee of 1,600 yen per request.
Please enclose fixed-amount postal money order in the amount of 1,600 yen only if you wish to file a request for access to Personal Information.

We do not accept any form of payment other than fixed-amount money orders.

Inability to comply with your request

If we are unable to comply with your request under the provisions of the law, or if we are not in possession of the Personal Information you have requested, a reply to that effect will be mailed to you (the Fee will not be refunded)

Incompleteness in documents submitted

If any inadequacy in the documents is found, the Personal Information Inquiry Desk will contact you. If you fail to make needed corrections or deliver additional documents within one month, we deem that no request has been filed.

Handling of submitted Personal Information

The Personal Information submitted in conjunction with your request will be used only to the extent needed for the work carried out in connection with your request. The submitted documents are not returnable.

Complaints regarding the handling of Personal Information
